Children and Adolescent Mental Health Services: follow up review of safey issues

This report has been prepared jointly by Healthcare Inspectorate Wales and the Wales Audit Office, and is presented by the Auditor General for Wales to the National Assembly under the Government of Wales Acts 1998 and 2006.

Our overall conclusion is that whilst there has been some progress by the Welsh Government and health boards in addressing the safety issues highlighted in our 2009

report, children and young people continue to be put at risk due to inappropriate admissions to adult mental health wards, problems with sharing information and acting upon safeguarding duties, and unsafe discharge practices.
